Job summary

St. Croix Hospice is seeking an experienced RN Case Manager to lead each patient’s hospice care team and develop their Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.

The RN Case Manager delivers daily patient care, manages treatments, maintains records, and directs other care team members to meet evolving needs with comfort and quality of life in mind.

Qualifications

• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Nurses holding a multistate license can practice in other states without additional licenses within PSOR.
• Graduate of a professional nursing program and licensed/certified in practice states.

Responsibilities

• Leads hospice care team and manages Plan of Care with Hospice Medical Director, patient, family and staff.
• Provide day-to-day patient care and administer medications per physician plan.
• Maintain accurate clinical documentation of pain, symptoms and patient changes.
