Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, coordinate and deliver in-home hospice care, educating patients and families while ensuring comfort and dignity. The RN conducts comprehensive assessments, develops care plans, administers medications and collaborates with the interdisciplinary team to meet patient goals within home settings.

Overview: strong communication, clinical judgment, and ability to work independently in patients' homes are essential.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum of one year of professional nursing experience in last three years or BSN with two years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing assessment of patient/family to determine hospice needs.
• Provide professional nursing care using the nursing process and administer treatments as prescribed.
• Develop and revise care plans with patient and family involvement.
• Communicate with physicians and interdisciplinary team; provide education and counseling.
• Coordinate care, participate in on-call duties, ensure equipment and supplies are available.
• Supervise hospice aides as required; participate in quality improvement activities.
