Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will use the nursing process to assess, plan, implement, and evaluate care, involving patients and families in goal setting and care planning.

The role emphasizes community health education, coordination with interdisciplinary teams, on-call duties, and ongoing professional development to ensure high-quality hospice services.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in Virginia.
• At least 1 year nursing experience in the last 3 years or a BSN with 2 years' experience, with hospice/home/public health experience preferred.
• Experience in public health, home care, or hospice nursing preferred.
• Criminal background checks required after offer.
• Excellent observation, verbal and written communication skills; problem solving; basic math.
• Must be able to lift, position and transfer patients; valid driver's license and insured automobile.

Responsibilities

• Complete initial, comprehensive, ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Initiate and revise the plan of care as patient needs change.
• Administer medications and treatments as prescribed by the physician.
• Counsel patients and families on nursing and related needs.
• Provide and maintain health care instructions to patients.
• Assist patients with activities of daily living and support self-sufficiency and comfort care.
• Maintain accurate clinical notes and document changes.
• Coordinate with physicians and interdisciplinary team; participate in on-call duties.
• Supervise hospice aides when required; pursue ongoing professional development.
