Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, direct, and deliver hospice care in patients’ homes and communities. The RN will assess, coordinate, and implement comprehensive care plans, educate families, and work with an interdisciplinary team to meet patient needs and comfort goals.

Qualifications

• Graduate of NLN accredited school of nursing.
• Current licensure in state (Ohio).
• RN with minimum 1 year of experience in the last 3 years; or a BSN with RN license and at least 2 years in public health, home care, or hospice nursing preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Conduct initial, ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and develop a care plan.
• Monitor patient status and revise care plans as patient needs change.
• Administer medications and treatments as prescribed and educate patient/family on care.
• Participate in on-call duties and coordinate with interdisciplinary team to ensure quality hospice care.
