Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. The nurse will base care on assessments, develop plans with patients and families, and coordinate interdisciplinary care.

Responsibilities include administering medications, counseling families, and acting as Case Manager when needed. On-call duties and supervision of aides are part of this role, with ongoing professional development expected.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with 1 year of experience within the last 3 years or BSN with 2 years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Completes comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with patient and family involvement; initiates plan of care.
• Administers medications and treatments as prescribed by the physician.
• Counsels patients and families; provides health care instructions as appropriate.
• Acts as Case Manager when assigned and coordinates patient care.
• Participates in on-call duties and ensures equipment/services are available.
• Supervises ancillary personnel and participates in quality improvement activities.
