Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize and direct hospice care for patients in home and community settings, leveraging nursing expertise and community health education to meet family needs.

Responsibilities include comprehensive assessments, care planning, medication administration, patient/family education, and coordination with the interdisciplinary hospice team. On-call duties and supervision of aides may be required.

Qualifications

• Graduate of NLN-accredited school of nursing.
• Current licensure in State.
• RN with 1 year of professional nursing experience in last 3 years OR BSN with current RN license.
• Preferred: 2+ years in public health, home care, or hospice nursing.

Responsibilities

• Complete comprehensive initial and ongoing patient/family assessments.
• Develop and revise care plans; initiate revisions as patient needs change.
• Administer medications and treatments per physician orders.
• Counsel patient and family; provide health care instructions.
• Coordinate care with hospice interdisciplinary team and act as case manager when assigned.
• Document clinical notes and report changes to physician.
• Participate in on-call duties and ensure availability of equipment/services.
• Supervise ancillary personnel as needed.
