Job summary

Gentiva Hospice is seeking a Registered Nurse (RN) Case Manager for full-time, Monday through Friday daytime shifts in the Hartford, CT area. The role involves leading patients and families through the hospice journey, delivering skilled nursing care at home or in alternative settings, and coordinating with an interdisciplinary team.

You will assess patient needs, document plans of care, and support families with education and bereavement guidance.

Qualifications

• Current nursing license in CT or eligibility for CT licensure.
• Minimum of 1 year RN experience; hospice or home health preferred.
• CPR certification required.
• Ability to perform home visits and document in electronic records.

Responsibilities

• Serve as primary point of contact for patients and families throughout hospice care.
• Deliver and document skilled nursing care per the Plan of Care in home or facility.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to refine personalized care plans.
• Provide education, support, and bereavement guidance to families.
• Educate caregivers on safe patient care and practical routines.
• Communicate updates to the care team for timely interventions.
• Maintain accurate clinical documentation and infection control standards.
• Support quality improvement and scheduling initiatives.
• Assist with end-of-life discussions and caregiver training.
