Job summary
Gentiva Hospice is seeking a dedicated Registered Nurse (RN) to join our team for full-time dayshift coverage in Northern Kentucky. You will perform comprehensive assessments, deliver skilled nursing care at patients' homes or facilities, and coordinate with a multidisciplinary team to implement personalized plans of care.

Qualifications
- Registered Nurse with current license in the state of employment.
- Minimum 1 year RN experience in hospice, home health, or related field preferred.
- Ability to educate patients and families on end-of-life care and medications.
- Proven skills in documentation, care planning, and compliance with regulations.
- Comfort working in patients' homes and coordinating with IDG.
Responsibilities
- Perform comprehensive patient assessments and follow-up visits.
- Deliver skilled nursing interventions and symptom management.
- Develop and update plans of care with visit frequency and risk identification.
- Coordinate care with physicians and interdisciplinary team.
- Educate patients and caregivers on disease, self-care, and end-of-life care.
- Document clinical records, orders, and progress notes timely.
