Job summary

Advanced-Health-Care-6 is seeking a Hospice RN Case Manager to coordinate and manage patient care in a hospice setting. Responsibilities include pain and symptom management, care plan development, and collaboration with an interdisciplinary team to ensure patient comfort and dignity.

This full-time role requires RN licensure, strong clinical skills, and the ability to work independently. Schedule is full-time, weekdays, 9 AM–5 PM, with home visits as needed; CPR certification and regulatory

Qualifications

• Current RN license in the state of practice.
• 1 year preferred hospice experience.
• Minimum of one year of RN experience in hospice or related care setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct comprehensive patient assessments to identify healthcare needs and develop individualized care plans.
• Provide skilled nursing care including pain and symptom management, medication administration, and patient education.
• Coordinate and supervise hospice aides and other professionals involved in patient care.
• Communicate with patients, families, physicians and team about progress and plans.
• Monitor patient well-being and comfort during end-of-life care.
• Collaborate with the interdisciplinary team to implement care aligned with patient wishes.
