Compassus is seeking a Hospice Registered Nurse / RN Case Manager to deliver high-quality patient-centered care at our Hartford, CT location. You will coordinate care plans, support families, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life for patients in their homes and communities.

Requirements include an active RN license in the state, at least two years of RN experience (hospice preferred), a valid driver’s license, and CPR/BCLS certification.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred.
• Current driver’s license
• Current CPR and BCLS certification
