Job summary

Gentiva Health Services, Inc. seeks a Hospice RN / RN Case Manager to lead in-home hospice care, guiding patients and families, and delivering skilled nursing at home or in supported living settings.

You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to build personalized plans and ensure compassionate care. The role emphasizes comprehensive assessments, care planning, symptom management, caregiver education, timely documentation, and adherence to regulatory

Qualifications

• RN with current license in the state of employment.
• CPR certification required.
• Experience in hospice, palliative care, or home health preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver and document skilled nursing care per Plan of Care.
• Develop and update individualized care plans and visit frequency.
• Coordinate care with an interdisciplinary team and physicians.
• Educate patients and caregivers on disease, medications and end-of-life care.
• Communicate changes in condition to the care team promptly.
• Document clinical records and maintain infection control compliance.
• Participate in on-call and patient support activities.
