Job summary

Gentiva, a hospice and palliative care provider, seeks a Registered Nurse to join as a Hospice RN / RN Case Manager, FT, dayshift, Monday through Friday. You will deliver hands-on nursing care in patients' homes or supported living settings in alignment with individualized plans of care.

The role requires at least 1 year of RN experience (hospice or home health preferred), current nursing license, CPR, and valid driver’s license.

Qualifications

• Active RN license in the state of employment (or eligible).
• CPR certification.
• Minimum 1 year RN experience in hospice, home health or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver skilled nursing care per Plan of Care in home, assisted living, or patient residence.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with interdisciplinary team to tailor and refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones; educate caregivers.
