Job summary

RVNAhealth is seeking a Registered Nurse (RN) Hospice Case Manager to join our hospice team. You will provide direct patient care and comprehensive case management for individuals in their homes or care facilities, coordinating care plans with the IDG and physicians.

This role covers Southern Fairfield County, CT (Trumbull, Fairfield, Bridgeport) and requires travel within the service area; you will supervise home health aides, participate in on-call rotations, and support families with

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current RN license in the State of Connecticut or Compact license.
• 2+ years of nursing experience; hospice or home health experience preferred.
• Ability to work independently and respond effectively in crisis situations.
• Strong clinical judgment and interpersonal communication skills.
• Comfortable with electronic health records and mobile documentation.
• Valid driver's license and willingness to travel within service area/territory.
• Current CPR certification.
• Must complete RVNAhealth's hospice training program upon hire.

Responsibilities

• Conduct admissions, comprehensive assessments, and regular reassessments of hospice patients.
• Develop, implement, and revise patient-centered care plans with the IDG and families.
• Deliver direct skilled nursing care and manage end-of-life symptoms in homes, SNFs, ALFs, and hospitals.
• Monitor condition changes and communicate updates to care team and physician; take action as needed.
• Support families with education, dietary guidance, and compassionate end-of-life counseling.
• Maintain accurate documentation in compliance with policies and Medicare/CHAP standards.
• Participate in IDG meetings and case reviews; ensure HIPAA and safety compliance.
• Supervise home health aides as part of the patient's care plan.
• Participate in on-call rotation and provide after-hours/weekend coverage.
