Job summary

RVNAhealth is seeking a Registered Nurse (RN) Hospice Case Manager to join our hospice team. The role focuses on direct patient care and comprehensive case management for patients receiving hospice services in homes or care facilities throughout Southern Fairfield County, CT (Trumbull, Fairfield, Bridgeport).

Responsibilities include admissions, assessments, care plan development with the IDG, and supervision of home health aides, while adhering to HIPAA and Medicare/CHAP standards.

Qualifications

• Graduate of an accredited nursing program. BSN preferred.
• Current RN license in CT or Compact license.
• 2+ years nursing experience; hospice/home health preferred.
• Strong clinical judgment and interpersonal communication skills.
• Comfortable with electronic health records and mobile documentation.

Responsibilities

• Conduct admissions, comprehensive assessments, and regular reassessments of hospice patients.
• Develop, implement, and revise patient-centered care plans with IDG and families.
• Deliver direct skilled nursing care and manage end-of-life symptoms at homes, SNFs, ALFs, and hospitals.
• Monitor condition changes and communicate updates to care team and physician.
