Job summary

Hartford HealthCare at Home Norwich is seeking a Weekday Primary RN Case Manager to support patients and families at end of life.

You will perform comprehensive assessments, establish goals for symptom management, collaborate with an interdisciplinary team, and document skilled services using state-of-the-art technology. A CT RN license and homecare or hospice experience are preferred.

Qualifications

• Active CT RN license required.
• At least 1 year RN experience; homecare or hospice preferred.
• Valid driver’s license and reliable transportation.
• Computer proficiency required.

Responsibilities

• Support patients, families, and loved ones through the end-of-life journey.
• Perform comprehensive assessment to establish goals for symptom management and end-of-life care with effective case management strategies.
• Be a member of an active interdisciplinary team that addresses patients’ needs holistically.
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
