Qualifications

• RN license in state of employment.
• Minimum 1 year RN experience in acute care, hospice, or home health; hospice/palliative care preferred.
• CPR certification; valid driver's license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and ongoing RN evaluations in home or patient locations.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update plans of care with visit frequency and risk identification.
• Coordinate care with physicians and IDG; educate patients and caregivers.
• Document all clinical records timely and maintain compliance with regulations.
