Job summary

Qualifications

• RN license in the state of employment.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per each Plan of Care at home, in assisted living, or other settings.
• Perform initial and ongoing assessments, monitor condition, adjust care as needed.
• Collaborate with IDT to build personalized care plans.
• Educate caregivers, provide bereavement guidance, and support families.
• Teach caregivers practical care techniques and emotional reassurance.
• Communicate changes in condition to the care team for timely interventions.
• Maintain accurate clinical records and follow infection control standards.
• Support quality improvement, scheduling, and care transitions across levels.
• Participate in on-call activities and coordinate care with the team.
