Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to lead skilled nursing care within patients' homes, guiding families through the hospice journey with dignity and empathy.

You will perform comprehensive assessments, create individualized care plans, coordinate with the IDG team, and provide education and bereavement support to patients and loved ones, ensuring timely, high-quality comfort-focused care.

Qualifications

• RN degree from an accredited program and active nursing license.
• Minimum 1 year of RN experience in hospice, home health, or related settings is preferred.
• Strong communication and education skills for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans and visit schedules.
• Coordinate care across the IDG and consult with physicians and other staff.
• Educate patients and caregivers on disease, care techniques, and end-of-life processes.
• Document clinical records accurately and maintain regulatory compliance.
• Participate in on-call and patient support activities as applicable.
