Job summary

Gentiva Hospice seeks an experienced RN Case Manager to lead in-home and facility-based hospice care. You will assess patients, implement plans of care, and collaborate with physicians, social workers, and chaplains to support patients and families with dignity and comfort.

You will educate caregivers, monitor symptoms, and coordinate with the IDG to ensure timely interventions and high-quality care in home settings.

Qualifications

• Registered RN with current state license.
• Minimum 1 year RN experience in hospice, palliative care, or home health.
• Hospice or palliative care experience preferred.
• CPR certification is required.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with IDG and physicians to refine care plans.
• Educate patients and families on disease, self-care, medications, and end-of-life care.
