Gentiva Hospice is seeking an RN Case Manager to lead patient-centered care in home settings. You will perform comprehensive assessments, implement plans of care, and coordinate with physicians and the IDG to refine treatment.

The role emphasizes education for patients and families and meticulous documentation of all care activities. Responsibilities include on-going assessments, coordinating with caregivers, and ensuring regulatory compliance while delivering compassionate end-of-life care at

Qualifications

• Must be a licensed RN in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or acute care.
• CPR certification required.
• Ability to work independently in patient homes.
• Strong communication and team collaboration skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing care per Plan of Care in home settings.
• Coordinate with IDG and physicians to update care plans.
• Educate patients and caregivers on disease, medications, and end-of-life care.
• Document clinical findings and ensure compliance with regulations.
