Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to lead patient care at home or in alternate settings. You will assess, plan, implement, and document nursing care aligned with each patient’s plan of care.

Work with an interdisciplinary team to coordinate services, educate families, provide bereavement support, and ensure timely communication and accurate records. Eligible candidates hold an active RN license, CPR certification, and hospice or palliative care experience; mileage

Qualifications

• Active RN license in requested employment state.
• CPR certification.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in relevant settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or other settings.
• Deliver nursing interventions and symptom management per care plans.
• Develop and update plans of care with visit frequency.
• Coordinate care with IDG and physicians.
• Educate patients, families, and caregivers on disease, self-care, end-of-life care.
• Document timely clinical records and ensure regulatory compliance.
• Participate in on-call activities as applicable.
