Gentiva Hospice in Myrtle Beach, SC, seeks an experienced Registered Nurse (RN) to lead in-home hospice care and coordinate with an interdisciplinary team to deliver compassionate, evidence-based care. You will assess, plan, and document patient needs while supporting families through difficult times.

The role emphasizes education, symptom management, and ensuring regulatory compliance across settings, including homes and care facilities.

Qualifications

• Active RN license in the state where employed; CPR certification required.
• Minimum 1 year in RN role; hospice or palliative care experience preferred.
• Ability to coordinate with interdisciplinary teams and document care plans.
• Excellent communication and empathy for patients and families.

Responsabilités

• Lead comprehensive in-home assessments and develop plans of care.
• Deliver skilled nursing care and symptom management.
• Educate caregivers and patients on caregiving tasks and disease processes.
• Maintain timely clinical documentation and comply with regulatory standards.
• Participate in on-call rotations as applicable.
• Collaborate with IDG members including physicians, social workers, chaplains.
