Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to provide skilled nursing care in patients’ homes, assisted living, or other settings of care. You will assess, plan, and implement individualized care, collaborating with physicians, LPNs, CNAs, social workers, and chaplains to ensure comfort and quality of life.

You will educate families, manage documentation, and participate in on-call coverage, helping patients navigate end-of-life options with empathy and professionalism.

Qualifications

• RN license in the state of employment
• CPR certification
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and education for patients, families, and caregivers.
• Provide skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with appropriate visit frequency.
• Coordinate care across interdisciplinary teams with physicians and other providers.
• Educate patients and caregivers regarding disease process, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Communicate changes in condition to the care team and ensure timely interventions.
• Support transitions and referrals across home health, palliative, and hospice services.
• Document timely clinical records including screenings, orders, progress notes, and care plans.
• Maintain strong IDG relationships and involve the team as needed.
• Participate in on-call and patient support activities with clinical responsiveness.
