Zusammenfassung

Gentiva Hospice is seeking an RN Case Manager to lead patient care in the home setting, guiding families through the hospice journey. You will perform comprehensive assessments, document care, and develop individualized plans of care to meet patient needs.

You will collaborate with an interdisciplinary team to provide compassionate nursing, education, and support, ensuring timely communication, accurate records, and adherence to regulatory standards.

Qualifikationen

• Requires active RN license in applicable state
• Minimum 1 year RN experience in acute care, hospice, home health, or related setting
• CPR Certification required
• Hospice or palliative care experience preferred

Aufgaben

• Perform comprehensive patient assessments and reassessments, delivering education to patients, families and caregivers.
• Provide skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and determine visit frequency.
• Coordinate care with an interdisciplinary team including physicians and specialists.
• Educate patients and caregivers about disease processes, end-of-life care, medications, nutrition and self-care.
