Job summary

Gentiva in Spartanburg, SC is seeking an experienced Hospice RN / RN Case Manager to lead patient care in home settings. You will perform comprehensive assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, social workers and chaplains to deliver compassionate, evidence-based hospice services.

You will educate patients and families, manage medications and symptom control, document care accurately, and participate in on-call coverage.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of 1 year RN experience, hospice or home health preferred.
• Hospice/palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care in home or facility settings per Plan of Care.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers, chaplains on care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers, document care, and maintain infection-control standards.
• Participate in on-call and patient support activities as applicable.
