Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead care at patients’ homes, assisted living, or wherever they call home. You will assess, plan, and deliver skilled nursing care aligned with each Plan of Care and collaborate with an interdisciplinary team to refine personalized care.

This full-time role runs weekdays 8am-5pm with a limited after-hours on-call rotation; you’ll educate families, document care, and support end-of-life care with dignity and comfort.

Qualifications

• Must hold a current RN license in the state of employment.
• Minimum of 1 year RN experience, hospice or palliative care preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive RN assessments and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care and visit frequencies.
• Coordinate with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate on disease, end-of-life care, medications, nutrition, and self-care.
• Document timely clinical records and maintain infection control compliance.
• Participate in on-call and care coordination activities.
