Job summary

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State of Ohio.
• Registered nurses shall have a minimum of one year of experience as a professional nurse within the last three years; or have a baccalaureate degree in nursing and a current RN license. Two years’ experience in public health, home care, or hospice nursing is preferred.

Responsibilities

• Initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Develops and revises plan of care with patient and family/caregiver.
• Administers medications and treatments as prescribed by the physician.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Coordinates with the interdisciplinary group to meet emotional needs and ensure continuity of care.
