Job summary

Affinity Hospice Management LLC in Ohio seeks a Registered Nurse to plan, organize, and direct hospice care delivered in patients' homes and communities. The RN will rely on community resources to meet the needs of individuals and families, emphasizing education and compassionate nursing practice.

You will perform comprehensive assessments, develop and adjust the care plan, coordinate with the interdisciplinary team, supervise aides, and participate in on-call duties.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice in the state.
• 1 year of professional nursing experience (last 3 years) or BSN with 2+ years in public health/home health/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Criminal background checks and references required.
• Excellent observation, verbal and written communication, problem solving, and math skills per competency checklist.
• Must be able to lift, position and transfer patients; valid driver with insured vehicle.
• CPR certification required (hands-on preferred).

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Initiate care plans and update as patient needs change.
• Document clinical notes and report changes to the physician.
• Coordinate care with the interdisciplinary team; participate in on-call duties.
• Supervise hospice aides and ensure equipment and services are available.
• Maintain a safe environment and continuity of care for patients.
