Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings across Ohio. The RN will use nursing processes to assess patients, develop and revise plans of care, and coordinate interdisciplinary teams to meet patient and family needs.

The role emphasizes patient education, medication administration, and collaboration with physicians and caregivers to ensure safe, effective home-based hospice services.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure as a registered nurse in the state.
• Minimum of 1 year of professional nursing experience within the last 3 years; or a BSN with RN license and 2 years’ experience (preferred in public health/home hospice).
• Supervisory experience not required; responsible for supervising hospice aides when needed.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Assess and revise the plan of care as patient status changes; initiate and modify care plans.
• Use health data to identify nursing diagnoses and establish goals with patient/family involvement.
• Deliver medications and treatments as prescribed and counsel patient/family on care.
• Educate patient and family on self-care techniques and health instructions as ordered.
• Assist with activities of daily living and support patient self-sufficiency and comfort.
• Act as Case Manager when assigned and coordinate care for the caseload.
• Maintain accurate clinical notes and communicate changes with physicians and team.
• Participate in on-call duties and ensure equipment and services are available.
