Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver compassionate, home-based hospice and palliative care in California. You will assess patients, develop plans of care, coordinate with an interdisciplinary team, educate families, and provide hands-on nursing support in patients' homes or alternative settings.

Current RN license in the relevant state.
Minimum of 1 year RN experience in hospice, home health, or related settings.
Hospice or palliative care experience preferred.
CPR certification.

Responsibilities

Perform comprehensive patient assessments and reassessments and provide education to patients, family members and caregivers.
Deliver skilled nursing interventions and symptom management.
Develop and update individualized plans of care with visit frequencies.
Coordinate care across interdisciplinary teams.
