Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home and facility-based care for patients at the end of life. You will perform comprehensive assessments, implement skilled nursing interventions, and update plans of care while coordinating with an interdisciplinary team including physicians, social workers, and chaplains.

You will educate families, provide emotional support, and ensure accurate documentation and regulatory compliance.

Qualifications

• RN licensure in the state of employment.
• Hospice/palliative care experience preferred.
• Knowledge of terminally ill patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver nursing interventions and symptom management per individualized plan of care.
• Coordinate care with IDG members and physicians for goal attainment.
• Educate patients and caregivers on disease, care, and end-of-life processes.
