Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to coordinate comprehensive in-home hospice care, providing skilled nursing and education to patients and families in their homes or approved care settings.

You will partner with physicians, LPNs, CNAs, social workers and chaplains to tailor plans of care, monitor patient status, and ensure compassionate end‑of‑life support. A valid RN license and hospice experience are preferred.

Qualifications

• RN license required in state of employment.
• Hospice or palliative care experience preferred.
• Strong communication with patients, families, and team.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver and document skilled nursing care per Plan of Care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams.
• Educate patients and caregivers on disease, care, and end-of-life topics.
• Communicate changes in patient condition to the care team.
• Document timely clinical records and ensure regulatory compliance.
• Participate in on-call and support activities as applicable.
