Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to deliver comprehensive nursing care in patients’ homes or care facilities. You will guide families through the hospice journey with empathy, assess needs, manage symptoms, and coordinate with an interdisciplinary team to ensure comfort and quality of life.

Qualifications

• Registered Nurse with GA license.
• Minimum of 1 year RN experience; hospice or palliative care preferred.
• Current nursing license in the employment state; CPR certification.
• Auto insurance and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease process, end‑of‑life care, medications, and nutrition.
• Document clinical records timely and maintain regulatory compliance.
• Support IDG activities and care transitions across levels of care.
