Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home settings and other living environments. You will deliver hands-on nursing care guided by individualized plans of care, reassess patient needs, and coordinate with an interdisciplinary team to ensure compassionate, compliant care.

The role emphasizes education for patients and caregivers, timely documentation, and participation in on-call activities as needed.

Qualifications

• Valid Registered Nurse license in the applicable state.
• Minimum of 1 year experience as an RN, hospice or home health preferred.
• Ability to perform comprehensive patient assessments and develop plans of care.
• Strong communication and education skills for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care with physicians, LPNs, CNAs and social workers.
• Provide patient and caregiver education on disease, care, medications and nutrition.
• Communicate condition changes to the care team.
• Support continuity of care across home health and hospice referrals.
• Document timely clinical records and maintain infection control standards.
• Participate in on-call and patient support activities as applicable.
