Job summary

Heart to Heart Hospice is seeking an RN Case Manager to assess and coordinate care for patients with life-limiting illnesses. You will implement personalized care plans and collaborate with a multidisciplinary team to ensure compassionate support for patients and families.

The role requires an RN license, a nursing degree, and a valid driver's license with reliable transportation. On-call duties and strict documentation are required as part of comprehensive hospice care.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License and reliable transportation.
• Two years of RN experience preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and ADLs.
• Implement the individualized POC and revise as necessary.
• Collaborates care among patient, caregivers, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning.
• Perform on-call responsibilities as assigned.
• Timely submission of all documentation.
• Ensure compliance with laws, Medicare regulations, policies.
