Job summary

Heart to Heart Hospice in Indiana seeks an RN Case Manager to coordinate comprehensive in-home/end-of-life care and lead patient-centered planning. You will perform ongoing assessments and tailor the care plan for patients with life-limiting illnesses, interfacing with families and the care team to ensure compassionate support.

The role requires RN licensure, a valid driver's license, and a willingness to engage in on-call duties.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborates with patient, caregiver, physicians and health professionals.
• Attend IDG meetings and coordinate patient care planning with team.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation and compliance with laws and policies.
