Job summary

Heart to Heart Hospice is seeking an RN Case Manager to assess and coordinate care for patients with life-limiting illnesses. The role emphasizes compassionate, patient- and family-centered planning and collaboration with a multidisciplinary team.

The position requires a nursing degree, an active RN license, and a valid driver's license with reliable transportation. Two years of RN experience is preferred. Comprehensive benefits are offered for a full-time role.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patient and family needs related to terminal illness and ADLs.
• Implement and revise the individualized Plan of Care (POC) as needed.
• Collaborate with patient, family, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning and prioritization with the IDG.
• Provide on-call coverage for patients and families as assigned.
• Timely submission of all documentation.
• Perform other duties as required and ensure compliance with laws, regulations, and policies.
