Job summary

Heart to Heart Hospice is seeking an RN Case Manager to lead comprehensive patient assessments and coordinate care plans for hospice patients. You will collaborate with physicians, caregivers, and the IDG to ensure timely, compassionate, and compliant care delivery.

The role requires an RN license and nursing degree or diploma, with preferred two years of RN experience in a clinical or healthcare setting. A valid driver's license and reliable transportation are required.

Qualifications

Two years of RN experience preferred in healthcare/clinical setting.
Current Driver's License and reliable transportation may be required.

Responsibilities

Initial and ongoing comprehensive assessments of patients' physical, psychosocial, and environmental needs and ADLs.
Implement and adjust the individualized plan of care.
Coordinate care among patient, family, physicians and healthcare professionals.
Attend IDG meetings and coordinate patient care planning.
Provide on-call service to patients and families as assigned.
Timely submission of all documentation.
Ensure compliance with applicable laws and policies.
