Job summary

Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care to individuals with life-limiting illnesses. You will perform comprehensive assessments, develop and adjust care plans, and coordinate with a multidisciplinary team to meet complex patient needs in the community.

The role requires an RN license and prior experience in healthcare settings, with responsibilities including on-call duties, documentation, and adherence to regulatory standards.

Qualifications

• Two years of RN experience preferred in a healthcare/clinical setting.
• Current State Registered Nurse License required.
• Current Driver's License with reliable transportation and valid auto liability insurance.

Responsibilities

• Conduct initial and ongoing assessments of the impact of terminal illness on patient needs and ADLs.
• Implement individualized care plans and revise as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning.
• Perform on-call responsibilities and provide on-call service as assigned.
• Ensure timely documentation and compliance with laws and policies.
