Job summary

Heartland Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in the home setting. You will perform comprehensive assessments, deliver nursing interventions, and coordinate with interdisciplinary teams to implement personalized care plans.

Responsibilities

Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and education for patients, families, and caregivers.
Deliver skilled nursing interventions and symptom management aligned with goals of care.
Develop and update individualized plans of care with appropriate visit frequency.
Coordinate care across interdisciplinary teams and consult with physicians.
Educate patients and caregivers on disease processes, self-care, and end-of-life care.
