Job summary

Heart to Heart Hospice is seeking an RN Case Manager to deliver compassionate hospice care, conduct comprehensive assessments, and coordinate interdisciplinary care for patients with life-limiting illnesses. The role emphasizes collaborative planning, on-call support, and strict documentation in a compliant setting.

The ideal candidate will be a registered nurse with a nursing degree, a valid state license, reliable transportation, and a commitment to compassionate service in the community.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care (POC) and revise as necessary.
• Collaborate care among patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning and IDG collaboration.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Ensure timely submission of all documentation.
• Perform other duties as required and ensure compliance with laws, Medicare regulations, and internal policies.
