Job summary

Heartland Hospice is seeking an experienced Hospice RN / Case Manager to lead care in patients' homes and care settings. You will perform comprehensive assessments, develop personalized plans, and coordinate with a multidisciplinary team to ensure compassionate end‑of‑life care.

RN license in the state of employment.
Minimum 1 year RN experience in acute care, hospice, or home health.
Hospice or palliative care experience preferred.

Responsibilities

Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments and education for patients, families, and caregivers.
Deliver skilled nursing interventions and symptom management aligned with goals of care.
Develop and update individualized plans of care with visit frequency and risk identification.
Coordinate care across interdisciplinary teams with physicians and nurse practitioners.
Educate patients and caregivers about disease, self‑care, end‑of‑life care, medications, nutrition and dietary needs.
Communicate changes in condition to the care team for timely interventions.
Support continuity of care and transitions between home health, palliative, and hospice services.
