Job summary

Heart’n Soul Hospice seeks a dedicated RN Case Manager to assess and coordinate end-of-life care for patients and families. You will implement and revise care plans, collaborate with a multidisciplinary team, and participate in IDG meetings to promote coordinated, compassionate care.

The role requires an active RN license and preferred 2+ years of RN experience in healthcare settings, with full-time on-site responsibilities and a strong commitment to patient-centered care.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the patient and family to determine needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings to coordinate care planning.
• Provide on-call coverage as assigned and ensure timely documentation.
• Maintain compliance with local, state and federal laws and Medicare regulations.
• Perform other duties as required.
