Job summary

Affinity Hospice Management LLC in Maine seeks a registered nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will use the nursing process to assess needs, develop care plans and coordinate with physicians and the interdisciplinary team.

You will perform comprehensive assessments, provide patient education, manage symptoms, and support families. This role includes on-call duties and driving to patient visits; licensure in good standing, CPR certification

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• Minimum 1 year of nursing experience in the last 3 years; or BSN with 2 years’ experience including public health/home care/hospice preferred.
• Management experience not required; supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication skills; basic math; nursing skills per competency checklist.
• Must be able to walk/stand for long periods; able to lift and transfer patients; lift supplies; physical stamina.
• Must be a licensed driver with insured vehicle.
• CPR certification required (hands-on).

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates plan of care and revises as patient needs change.
• Delivers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Delivers health instructions and supports patient self-care.
• Acts as case manager when assigned.
• Completes and submits accurate clinical notes; reports changes to physician.
• Coordinates care with the physician and community health professionals.
• Teaches self-care techniques and provides health counseling.
• Keeps patient safe and supports continuity of care.
• Participates in interdisciplinary group meetings.
• Participates in on-call duties and ensures necessary equipment and services.
• Supervises ancillary personnel and delegates tasks.
• Pursues ongoing education and professional development.
• Fulfills case assignments and quality improvement activities.
• Engages in quality assessment and performance improvement teams.
