Job summary

Affinity Hospice Management LLC is seeking a dedicated registered nurse to plan, organize and direct hospice care in home settings within Maine. You will provide comprehensive nursing care, assess patient needs, develop and revise care plans, and collaborate with an interdisciplinary team to support patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• 1+ year professional nursing experience within the last 3 years; OR BSN with minimum 2 years' experience in public health, home care, or hospice preferred.
• Management experience not required; responsible for supervising hospice aides.
• Employment contingent upon references and criminal background checks as required.
• Excellent observation, verbal and written communication skills, problem solving skills, and basic math skills.
• Must be a licensed driver with a vehicle insured and in good order.
• Possesses and maintains CPR certification (hands-on certification preferred).

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Assess and evaluate patient status; write and initiate plan of care; revise as needed.
• Implements and revises plan of care as patient needs change.
• Use health data to determine nursing diagnosis and goals; involve patient and family.
• Develop care plans incorporating palliative actions with patient/family involvement.
• Initiate preventive and rehabilitative nursing procedures; administer prescribed meds.
• Counsel patient and family in meeting nursing and related needs.
• Provide health instructions and support self-care for patients.
• Assist patient with activities of daily living to enhance comfort; support self-sufficiency.
• Act as Case Manager when assigned and coordinate care for caseload.
• Attend interdisciplinary meetings and coordinate with team to meet emotional needs.
• Participate in on-call duties; ensure equipment and services are available.
• Supervise ancillary personnel and delegate tasks when required.
• Maintain professional knowledge through ongoing education and in-service training.
• Fulfill case assignments and engage in quality improvement activities.
