Job summary

Affinity Hospice Management LLC is seeking a registered nurse to plan, organize, and direct hospice care in home and community settings. The RN will conduct comprehensive assessments, develop and implement a patient-centered plan of care, and coordinate services with an interdisciplinary team.

The role emphasizes education for patients and families, adherence to the nursing process, safe care delivery, and ongoing communication with physicians and caregivers.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in good standing.
• Minimum of 1 year professional nurse experience in the last 3 years; or BSN with 2+ years in public health, home care, or hospice preferred.
• Management experience not required; responsible for supervising hospice aides.
• CPR certification required (hands-on demonstration preferred).
• Excellent observation, verbal and written communication skills; problem solving; basic math skills.
• Must be able to lift and transfer patients and have driving license with insured vehicle.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing the nursing process.
• Writes and revises plan of care; evaluates patient and family needs.
• Initiates and revises the plan of care as patient status changes.
• Coordinates patient care as Case Manager when assigned by Clinical Supervisor.
