Job summary

Qualifications

• Graduate of an accredited school of nursing.

• Minimum 1 year nursing in last 3 years; or BSN with 2 years, at least one in public health/home care/hospice.
• CPR certification required.
• Valid driver’s license.

Responsibilities

• Complete initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and administer prescribed treatments.
• Develop and revise the plan of care with patient and family involvement and coordinate with the interdisciplinary team.
• Counsel patient and family, provide health instructions and support activities of daily living as needed.
• Participate in on-call duties and ensure equipment and services are arranged.
