Job summary

Affinity Hospice Management LLC in Maine is seeking a registered nurse to plan, organize and direct hospice care in home and community settings. The RN will assess patients, develop and revise plans of care, administer medications, and educate families to support comfort and quality of life.

The position requires a current RN license, CPR certification, and excellent communication. Experience in home or hospice nursing is preferred; valid driver's license and safe vehicle are expected to support

Qualifications

Graduate of an accredited school of nursing.
Active RN license in good standing.
Minimum 1 year of professional nursing experience in the last 3 years (or a BSN with 2 years' experience).
CPR certification required.
Excellent observation, verbal and written communication, and problem-solving skills.

Responsibilities

Assess patient and family to determine hospice needs and develop plan of care.
Provide nursing care and administer medications as prescribed.
Revise plans of care as patient needs change; coordinate with interdisciplinary team.
Educate patient and family on care techniques and disease management.
Maintain accurate clinical notes and ensure safety during home visits.
Participate in on-call duties and ensure necessary equipment and services are available.
