Job summary

Affinity Hospice Management LLC seeks a Hospice Registered Nurse Case Manager in Maine to plan and direct in-home hospice care, focusing on education and family support. The RN coordinates patient care, conducts comprehensive assessments, and collaborates with the hospice team to adapt care plans.

Licensed RN with one year of nursing experience is preferred; a $1000 sign-on bonus is available for eligible candidates.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Minimum of 1 year of nursing experience within the last 3 years; or a BSN with 2+ years in public health, home care, or hospice.
• Management experience not required; may supervise hospice aides.
• Criminal background checks required after offer.
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Conducts initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and initiates a plan of care; regularly re-evaluates patient and family needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnoses and creates care plans.
• Administers medications and treatments per physician's order.
• Counsels patient and family in meeting nursing needs and self-care.
• Provides health instructions and ensures safety in the patient’s environment.
• Assists with activities of daily living and supports patient self-sufficiency.
• Acts as Case Manager and coordinates care for assigned caseload.
• Participates in on-call duties and ensures equipment and services are available.
• Supervises ancillary personnel as needed and pursues ongoing professional development.
