• CPR certification required.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Develop and revise care plans; initiate plans as patient needs change.
• Maintain accurate clinical notes and communicate with physicians and team.
• Coordinate with interdisciplinary team; participate in on-call duties.
